Yep, I had it stuck in my head that the +3 was at 12 and I thought the planner was broken.  Also, it applies at the 'end' of leveling, which is where my question originated.

It's always one level before you can raise a attribute with a level up. If we had +5s, they would apply at level 19.

If you eat a +3 int tome and start your next life with an odd int, will the bonuses at 3 and 11 apply before you pick your skills (allowing extra points at those levels) or after?

I can confirm the int from a past life tome giving you an extra point while leveling up. I ended up with a point more than the character planner thought I would at 7.
