Dual boxing redwillow. Pulled a collapsed portable hole. Passed it in the box to the other toon....forgot to pick it up on that toon.

Good thing CPH's are fairly easy to get these days!

Back when the level cap was 10, I pulled a robe of wizardry VIII, a super rare item at the time.  I promptly handed it over to our group mage who just as promptly dropped it out of his inventory (apparently he tried to drag it to his actual character to equip it a la WOW).  He got his balls busted over that for a looooong time.

Killing undead. No problem hahaaaa!!! Then I see the big ones....hell, I am a cleric. No, a RADIANT SERVANT!!! I should be able to... *ding*... binding point in Smugglers Rest tavern...fuck...I have to bind in Jorasco... run to Jorasco tavern... ok, lets repair stuff... drink for fast sp recover...lets buff...something more? No, that was all.

Repeat from the beginning.
It took me about 6 or 7 times until I actually binded in Jorasco.

healing the tank in epic lob and then forgetting you have tank targeted when you goto heal youself = spl4t

Part 4, melee on Arry, I announce who is the center of my mass healing... and I heal using the single targetted spell instead of the mass one. Obvious results ensue.
*ding* *ding* *ding*  Oh, shit !  time. 

As it was before the Shade Changes we killed Arry ( 2 rounds ) with half the party dead.

I'm not sure what is worse, that when I finally brought in my cleric for the first time today in epic lords of dust I tried to heal the skellies to death, or that the party waited until after I had used all of my turns and gone through about 10 mass heals before telling me to stop healing the undead.

Hit wrong button and cast D-Door directly on top of him.
