Wiki only mentions what drops in the raid and what doesn't.

I'd like to make a list of locations for each item out there so I will start with what I've personally saw and will update as people share info. Please provide only first hand information and not just something you heard. If you can SS it then even better.

Items that have a heroic version will most likely drop at the same quest on epic but confirmation would be nice.

Also, Epic Taps seem to drop in the Wilderness area at a rate of 2 per red name (per person).  They go straight to inventory once you slay the red name.  Not seen any in chests so far.
Random encounters didn't seem to give any.

you use them to unlock the slots on items, 1 of them more specifically, the 2nd one is unlocked by using raid taps

150 Epic Taps for Tier 1 slot (Blue or Yellow)
120 Masterwork Taps for Tier 2 (Green Slot)

The raid on EN drops between 6-8 per Masterwork per end chest. EH I think it was 12ish, not sure of EE (yet)

I think all Emerald comes from Vol.  Fits thematically.

Pulled Emerald Twilight last night, think it was from Vol, but not sure.
Noxious Embers is a raid item in heroic, I'll leave it to you to put there if you want until confirmed.
