Most of what you need is earlier in this thread. Just change it to 17druid/2rog and 1 fighter or barb.

41-Falconry
21-Nature’s Warrior
12-Nature’s Protector (core 3 and roar stun)
4-fight haste boost 3
1-bonus
2(16)      - Aasimar-quick draw

First 3 levels Druid to get natural fighting quick.
4th level rogue to get skills. 
Take your martial level sometime before 12.
Final rogue somewhere towards the end to fill in skills or earlier if you need the evasion.

1-quicken
3-natural fighting
6-natural fighting
9-natural fighting
10-precision (fighter bonus feat)
12-improc crit
15- completionist
18- Optional (heighten, spell focus evoc, toughness or empower healing)
21-overwhelming crit
24-BoGW or epic toughness or great ability wis
26-Perfect 2 wpn or Guardian Angel (120 or so prr)
27-Epic dmg reduction or blinding speed, or ability wis
28-Mass Frog
29-Dire charge
30-Past life Wizard or previously untaken feat
30-Arborea, Air or Astral (all good in their own way)

The vid does a good job of showing it's damage, durability, cc ability, stealth ability, trapping, intimidate.
